[A] Budget: Looking For Proposal
[B] Scope of Service:
(1) Vendor needs to provide wellness campaigns and challenges services for challenges, social networking, educational content on health and wellness, fitness tracking and rewards through a raffle based on completed activities within the portal
- population health management for health risk assessment, biometric screening, coaching, health management/intervention and participation tracking for incentive purposes and
- clinic service for preventive care, immunizations and injections, health and nutrition coaching, coordination with the state of population health management programs to provide education and intervention programs, lab tests, immediate/acute care, prenatal services, behavioral health services, dispensing of prescription drugs and face to face counseling services that can be billed through the EAP program with anthem to the government authority located in Indianapolis, IN.
(2) All question must be submitted in written no later than April 20, 2018.
[C] Eligibility:
Onshore (USA Only);
[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location.
